If your Student is planning to participate in NBCA Sports for the 2020-2021 school year, they must have a physical & medical history form done and turned in by the first day of practice. They will NOT be able to participate in practices or games until a current physical & medical history is on file with the Athletic Department.

Independent School Athletic League (ISAL) requires an annual physical & medical history for all Middle School students who participate in school sports. We recommend having this done over the Summer so the physical is current for the entire school year.

Click Here for access to the MS Physical & Medical History forms. The physical portion of the form must be filled out and signed by the Physician. The medical history portion should be filled out and signed by a parent and student. The standardized physical form from a Physician will be accepted also. All forms need to be read and signed by a parent and student.

Please turn in all Physical & Medical History and Athletic forms to the Athletic Office or at the Front Office of the Secondary Campus.

If your son or daughter has already turned in a physical dated May 2019 or after they are covered for this school year.
